Frequently Asked Questions


For your convenience, we have answered several questions that are commonly asked. If you have a question that is not listed or need further information about the ones that are listed, please ask one our staff members. We would like to thank you and appreciate your trust in Dr. Polson and staff!

Appointments
Appointments can be made anytime during scheduled business hours. If you are unable to make a scheduled appointment please call us as soon as possible so we can reschedule your appointment. Patients need to provide a photo ID and insurance card upon check in. We call patients as a COURTESY to confirm appointments a day to two days prior scheduled appointment.

Cancellations (Office or Procedure visits)
We ask that you give us twenty-four (24) hour notice to cancel office and/or procedure appointments. If our office does not receive a twenty-four (24) hour notice of cancellation you will be charged $80 for the missed office visit new or established patients and $100 for a missed procedure.

Procedure Scheduling
Per Medicare & State regulations if you are unable to have your procedure within 30 days of your office visit and physical exam, you will be required to schedule another office visit to update and repeat your physical exam before sedated procedure can be done. This policy is required due to government regulations.

Messages/Returned calls:
In most cases please allow up to 48 hours for a returned phone call. We take and receive messages in the order received. Most calls are returned same day but we ask that patients allow ample time to return your call, as doctor is in between clinic when messages are left or out of the office in procedures.

Prescription Refills
Please call you pharmacy at least three(3) business days before you need your medication refill. Often times medication refills may need authorization and/or require you to be seen before Dr. Polson will authorize a refill.

Results
We understand that often times you are told by the physician, radiology, lab, etc. to call our office in a couple of days for your results. We request that you allow 7 days to receive your results, as the entire process can take this long.

Courtesy
Please NO cell phone use in the building. We ask that you kindly handle all phone conversations before entering the building: cell phone calls can delay your appointment.

Disability Forms
There is a $35 charge for each Disability or Family Medical Leave Act (FMLA) form we complete on your behalf. Allow up to five business days to process your request.
